A GUIDE TO OBTAINING A PROVISIONAL
LGV DRIVING ENTITLEMENT

 

 
 

Provisional LGV entitlement must be obtained prior to an ON ROAD driving assessment, theory test or driving tuition can commence. To obtain this, follow the procedure below:-

  • Contact your doctor and arrange for an appointment for a medical examination. Your doctor will charge approximately £120.00 for conducting the examination and completing the Medical Form D4, alternatively let us orgainise an appointment on your behalf for the appropriate medical examinination and completion of the D4 form, this will be carried out at a local medical centre for a cost of £65.00

  • Complete the Application for Provisional Licence Form D2
  • Send all the items listed below together in one envelope.
a) Car Licence
b) Completed Application for Licence with original documents as required
c) Completed Medical Form

 

  • DVLA will normally reply to your application after approximately two to three weeks, by returning your new photo card driving licence and paper counter-part to include your provisional LGV licence entitlement. If there is a long delay, contact Customer Enquiries. DVLA Swanswa SA6 7JL or telephone 0870 240 0009 or fax them on 01792 783071.
